מבקרהמימצאים הללו המתוארים במחקר הזה נראים תמוהים משהו… ומכיוון שלמיטב ידיעתי נעשו לא מעט מחקרים שבדקו את היעילות של ממתיקים מלאכותיים בהפחתת משקל ושמירת רמות סוכר – איך זה יכול להיות ? האם זה אומר גם שכל מוצרי המזון שמקבלים את תוי האגודה לסוכרת אינם למעשה מומלצים באמת לחולי הסוכרת ?

ד"ר סיגל אילת-אדרלא הייתי שופכת את התינוק עם המים… מאחר והנושא עלה בעוד מקומות, אני מעתיקה את תשובותי ותשובות אחרים תחת שמם, מן הפייסבוק:

"זה מחקר מעניין. ראשית פורסם בעיתון מצויין, שנית למרות החלק הארוך של מחקרים בעכברים (יותר מ- 3/4 מהמאמר), הם בדקו גם 381 (מספר לא מבוטל) של אנשים. יחד עם זאת, למרות שנראה שלא, זה מחקר שבו לקחו נתונים מאנשים שאכלו ממתיקים מלאכותיים. ייתכן שאנשים אלה אוכלים ממתיקים מלאכותיים מסיבה בריאותית, או אולי סבלו בעבר מהשמנה ועברו לאכילת ממתקים. במקרה כזה זו סיבתיות הפוכה.
מאחר ומדובר באנשים בריאים ללא סוכרת, משמעות התוצאות לא ממש ברורה לי.
אולי הסתכלתי לא נכון, אבל לא הצלחתי לראות אם יש הבדל ב- BMI התחלתי בין אוכלי הסכרין לקבוצת ההשוואה. כמו כן בטבלה שאליה מפנים קיימות קורלציות אך לא רמות המשתנים השונים בבדיקות הדם. שכן גם אם קיימת עליה, תמיד נשאלת השאלה אם יש לה משמעות קלינית.
גם במחקר ב NewEng תיתכן סיבתיות הפוכה. אני פורשת מהדיון. השורה התחתונה היא שאני לא ממליצה לכן לתת למטופלים שרוצים לרדת במשקל להחזיר את הסוכר לשתיה וליוגורטים. בוודאי לא לחולי הסוכרת.

זאת למרות ההסתייגויות שלי מהכתוב, כפי שפרטתי לעיל וכן מכיוון שדבריה של קרן מבוססים על מחקרים תצפיתיים, שיכולים להראות על סיבתיות הפוכה "דיאט קולה משמין כי כל השמנים שותים דיאט קולה".
מאחר ובעלי הינו אחד החוקרים אשר הוביל את המחקר, דעתי לא לגמרי אובייקטיבית…. אך חשוב לזכור שהרבה מההנחיות התזונתיות שאנו שבויים בתוכן ומעניקים למטופלינו ללא עוררין, אינם נשענים על מידע מדוייק ומחקרים חד משמעיים. ראה נושא הנחיות לדל שומן של ארבעת העשורים האחרונים… אני מצרפת מספר מאמרים שנעשו על הקשר שבין ממתיקים מלאכותיים ומחלות מטבוליות.
אין ספק כי המאמר שהתפרסם בכתב העת היוקרתי nature הינו קצה הקרחון ודרושות עבודות נוספות בנושא. אך יחד עם זאת, מדובר בזעזוע גדול ומשמעותי לתעשייה של הממתיקים המלאכותיים בכלל וחולי הסכרת בפרט. ואם אכן ממתיקים מלאכותיים גורמים לשינויים במיקרוביוטה במעיים אשר מעלים תנגודת לאינסולין,עלינו להיות מאוד זהירים כאשר אנחנו אומרים למטופלינו שאין שום בעיה עם דיאט קולה או כל ממתיק אחר במוצרים שהם צורכים ללא מעקב.
